To facilitate the clients’ wish to remain independent and safely Age at Home, providing proactive monitoring, management, and coordination of all necessary health and well-being services through a dedicated nurse. My approach is holistic, aiming to integrate wellness and natured care. Life-est is committed to supporting independence, wellness, and peace at HOME. Life-est delivers ongoing comprehensive nursing care through a Health & Medication Management Program and addresses any nursing needs that arise. We specialize in creating customized Life Care Plans tailored to individual needs, educate wellness with the whole person by considering body, mind, and spirit, and developing a Family Plan to keep loved ones informed and involved. Should health concerns arise, your Nurse serves as the clients' personal advocate, expertly handling any health care needs, communicating and coordinating seamlessly with their primary care provider, other specialists, and family members. Our commitment to maintaining a small practice size allows us to provide our clients with the invaluable resources of personalized care, deep compassion, and dedicated time.
